Service Description:

A Discharge Book issued for seafarers working onboard and is given only to Qatari Seafarers.

Service procedures:

Filling the form and delivering it with attached requirements to the department staff who make sure the required documents are attached and then the Discharge Book is issued.

Fees:

• 100 QAR

Requirements:

• A formal letter from the company directed to Department Director.
• Copy of Marine Certificate.
• Seafarer Medical Fitness Certificate.
• Copy of the Passport.
• Copy of Police Clearance Certificate.
• (2) Colored Photographs.
• Fill out the form prepared by the administration.
